From April 2018 operators are required to report game faults resulting in under or overpayments to players as a key event under licence condition 15.2.1.

The Gambling Commission has recently updated its guidance about what type of gaming faults should be reported, what should be included in the key event, and what steps operators and/or suppliers should be taking to remedy a fault.
